PET CRISIS 365 RELEASE FORM

This Release Form (the "Agreement") is entered into as of the date signed below by and between Pet Crisis 365 ("Company") and the undersigned individual ("User"), who acknowledges and agrees to the following terms and conditions regarding the use of Pet Crisis 365 services.

1. Services Provided

Pet Crisis 365 offers guidance, support, and resources related to pet emergencies, welfare concerns, and crisis situations. The Company is not a substitute for professional veterinary care, emergency services, or law enforcement intervention.

2. Assumption of Risk

User acknowledges that handling pet crises may involve inherent risks, including but not limited to injury to oneself, others, or the pet. User voluntarily assumes all risks associated with acting on the advice or resources provided by Pet Crisis 365.

3. Release of Liability

User agrees to release, waive, and hold harmless Pet Crisis 365, its owners, employees, contractors, and affiliates from any and all claims, liabilities, damages, or losses that may arise from the use of the services, including but not limited to injuries, property damage, or adverse outcomes.

4. No Guarantee of Outcome

User understands that Pet Crisis 365 provides guidance and recommendations based on available information but does not guarantee specific results. It is the User’s responsibility to seek appropriate professional help when needed.

5. Medical and Emergency Disclaimer

Pet Crisis 365 does not provide veterinary or medical diagnoses, treatment, or legal advice. In cases of emergency, Users should contact a licensed veterinarian, emergency pet hospital, or local authorities as applicable.

6. Confidentiality and Data Use

User acknowledges that any personal information shared with Pet Crisis 365 may be stored for record-keeping and service improvement purposes but will not be shared with third parties without consent, except as required by law.

7. Indemnification

User agrees to indemnify and hold harmless Pet Crisis 365 from any claims, damages, or legal actions arising from User’s reliance on or implementation of any information provided.

8. Agreement Termination

Pet Crisis 365 reserves the right to terminate service to any User at its discretion, especially in cases of misuse or abuse of the services provided.

9. Governing Law

This Agreement shall be governed by the laws of the jurisdiction in which Pet Crisis 365 operates.

By signing below, User acknowledges they have read, understood, and agreed to the terms of this Release Form.
